Alias: What you like for people to call you
Age: I'm asking you this because some people aren't comfortable playing with people who are older or younger than they are. If you don't feel comfortable giving your age, then if you could just say over/under 18, that would be fine.
Gender: Same as age. Answer only if you want to. Leave blank, otherwise.

Personality: Describe your general personality. Are you introverted? Extroverted? Do the best you can.

About Me: Is there anything really interesting about yourself that you'd like people to know? Hobbies? Fandoms? etc.

Do you like to use messengers? Some people don't like to talk to people that they don't know in real life. Are you willing to IM someone you don't really know and strike up a conversation? What messengers do you generally use, if so?

How long have you been roleplaying?

What boards have you played on in the past? Just names, no links.

ABOUT ROLEPLAYING

How often do you generally post?: Some people like fast posters, some people like slow posters. By answering this question, you can give people a general idea of what to expect from you, should they get involved with you.

How much do you generally post? Some people don't like people who write a lot and some people don't like people who write a little. If you have an average word count, give it. If it varies from short to long, depending on certain variables, then say that.

Do you like to write in 1st person or 3rd person?

Who are your favorite PB's? What playbys do you generally use for your characters? Leave blank if you don't use playbys or don't have a specific list.

What PB's do you hate? Again, leave blank if you don't use PBs.

What type of characters do you like to play? Male or female? Happy? Loners? etc.

What are your favorite genres? Be as thorough as you can.

What rating of a board do you like to play on? PG, PG-13, R, NC-17 etc
How do you feel about writing out graphic scenes such as violence, drug use, and sex? Some people don't like to write such scenes at all and some people prefer to. By stating what you are comfortable with, you will avoid this awkward situation.

What type of plots do you like? Romance? Sworn Enemies? etc

What type of plots do you dislike?

What are some plots that you'd like to play out with someone? Again, be as thorough as you can.
